I would also check the back of the frame where the hammer stops to see if it is peened up. I have seen that one time before many years ago when a fellow dry fired w/o the firing pin and stop in position to practice his trigger pull. He had been told dry firing would damage of bread the firing pin. In doing so the hammer peened the back edge of the frame up and caused the wear pattern shown. Just a guess as it is hard to diagnose such problems positively just as Tim mentions.
